Thank you for filling out the Ministry Placement Profile. Your request for assistance will remain in effect for six (6) months from the date of submitting this form. You may contact the Placement Services Office at any time to change your status. If you wish to continue receiving assistance beyond a six-month period, you must complete and submit the Ministry Placement Profile again.

All information submitted will be kept confidential by the Placement Services staff.
